Forest Alcohol Drug Counseling Inc. is SUPR licensed and located in Fores Med-Surg Center in Justice, Illinois. our agency is fully staffed to accommodate a wide range of counseling needs for our clients.

Forest Alcohol Drug Counseling Inc. therapists and counselors are licensed and certified in the State of Illinois, and our agency is a licensed treatment program. Our counselors participate in advanced training yearly to insure that they are up to date on current, quality services.

We service all Illinois Counties. We are on service providers list for Cook County, DuPage County, as well as for persons sentenced to probation in DuPage County.

Carol Koeneman, LPC, CADC

Graduated from De PaulUniversity with a bachelor’s degreeand Governors State University with a master’s degree. She is qualified to complete comprehensive services for all levels of DUI, Court, Secretary of State services (i.e. Court & SOS evaluations, Substance Abuse Assessments, Denials, Non-DUI Evaluations.) As a practicing addictions counselor for over30 years Carol currently provides individual, group counseling, discharge planning, evaluations and assessment services, as well as risk education classes.

Fran Pukala, LPC, CADC

A graduate of Chicago State University with a bachelor and master’s degrees. Fran is also a certified addictions counselor that facilitates risk education classes, group counseling, intake assessment and discharge planning. Fran also did prior work at Sladie Waterford facility participating in individual counseling with children and families.

Carol Koeneman and Fran Pukala have over 40 plus years of service in this industry.
